Cagrilintide carries a Cys2–Cys7 disulfide, and almost everything analytically interesting about it follows from that. The reduced open-chain form at +2 Da elutes 1.2 min early and is the most common impurity in poorly handled lots; a scrambled isomer is mass-identical and only resolves on RG-2's full 30-min gradient. Because reduction is a handling artefact, the index flags any cagrilintide report where the open-chain form exceeds 1.5 % area for cold-chain review rather than counting it against the synthesis. Coverage is thin — n is under 90 across the whole index — so per-supplier medians for cagrilintide carry a low-n caveat everywhere they appear. Base peak [M+4H]4+ at m/z 991.34.

Cagrilintide is studied both alone and co-formulated with semaglutide as CagriSema; phase 2 data appeared in Lancet (2021). No approved product exists.
